Thyroid and heart failure: what should you know?

Too much thyroid hormone can make the heart beat faster and may contribute to palpitations or an abnormal rhythm. Too little thyroid hormone can slow body processes and may be associated with tiredness, exercise intolerance and changes in fluid handling. Heart failure means the heart is not pumping or filling effectively enough for the body's needs; symptoms can include breathlessness, cough, fatigue and swollen ankles. These symptoms are not specific, and thyroid disease does not by itself establish heart failure.

A clinician may review your pulse, blood pressure, weight trend, medicines and medical history, then arrange thyroid blood tests and heart-focused investigations when indicated. Rhythm problems such as atrial fibrillation can complicate assessment and deserve specific attention.

Book a review if you have a diagnosed thyroid condition alongside breathlessness, swelling, reduced exercise tolerance or repeated palpitations. Take medicines exactly as prescribed and keep a record of pulse symptoms, weight changes, breathlessness and ankle swelling. Avoid changing thyroid or heart medicines independently. A chronic disease review can help coordinate thyroid monitoring, heart assessment, blood pressure care and medicine checks.

Seek emergency help for severe or rapidly worsening breathlessness, chest pain, fainting, blue lips, new confusion or a fast irregular heartbeat with weakness or dizziness. Prompt medical review is also needed when swelling increases quickly, you cannot lie comfortably because of breathing difficulty, or symptoms follow a medicine change.
