Testosterone and hair loss: what to know

Testosterone can be converted in the skin to a stronger androgen. Genetically sensitive follicles may gradually produce finer, shorter hairs when exposed to that signal. On the scalp, this can look like recession at the temples, thinning at the crown, or a wider part. On other body areas, androgens may increase hair growth instead. The same hormone therefore does not create one uniform pattern.

Male pattern hair loss and female pattern hair loss are possible patterns, while telogen effluvium causes more general shedding. Alopecia areata may create smooth patches, and scalp ringworm can cause scaling or broken hairs. Traction alopecia is linked to repeated pulling rather than testosterone.

Book an examination when loss persists, progresses, or is affecting your confidence. A clinician will look at distribution, hair-shaft changes, scalp inflammation, and follicle openings, then review family history, recent illness, weight change, medicines, supplements, and grooming habits. Blood testing may be useful when symptoms point to a hormone, iron, thyroid, or nutritional issue, but it is not a universal first step.

Avoid self-prescribing testosterone-altering medicines. Discuss treatment options, including whether finasteride therapy is suitable, with a clinician who knows your medical history. Protect remaining hair from tight styles, harsh chemicals, and repeated high heat.

Get prompt assessment for sudden loss, rapidly enlarging patches, scalp tenderness, pus, intense itching, thick scale, or broken hairs. Seek timely medical advice for hair loss with new acne, increased facial or body hair, voice change, or an unusual change in periods. Fever, spreading redness, or a painful swollen scalp warrants urgent attention.
