How the symptoms can connect

Shoulder tension often follows prolonged computer work, carrying a bag, repetitive activity, poor rest or stress. The upper shoulder and neck muscles share movement and support duties, so tightness can spread upward and produce pain around the temples, forehead or back of the head. A headache with fever, blocked nose, facial pressure or feeling unwell may instead occur alongside an infection or another condition; the headache does not establish which condition is present.

Consider recent heat exposure, low fluid intake, anxiety, allergy symptoms and illness. Record whether movement, rest, warmth or changing your workstation alters the pain.

Relieving shoulder-related strain

Change position regularly and let your shoulders drop away from your ears. Use gentle shoulder rolls and comfortable neck movement, stopping if pain spreads or becomes sharp. A warm shower or heat pack may ease muscle tightness. Adjust screen height so you do not lean forward, keep frequently used items close, and vary which shoulder carries a bag. Drink normally and rest. Follow medicine labels and ask a pharmacist if pain relief is unsuitable for you.

When to seek medical care

Seek urgent help for a sudden severe headache, weakness or numbness, difficulty speaking, confusion, fainting, seizure, new vision loss, persistent vomiting, or fever with a stiff neck. Arrange a GP assessment if headaches keep returning, interfere with sleep or work, follow an injury, or do not improve after changing the provoking activity. Mention fever, nasal symptoms, dehydration signs or ongoing worry so the clinician can consider the full picture.
