Why thighs may swell

After a caesarean birth, intravenous fluids, hormonal changes, reduced walking, and pressure on pelvic veins can allow fluid to collect in the legs and thighs. Bruising, muscle strain, or a tight dressing can also make one area feel fuller or tender. Mild swelling on both sides that improves with position changes may settle as mobility returns.

A clot in a deep vein is a separate concern. It may cause swelling, aching, warmth, or colour change in one leg, although symptoms are not always obvious. High blood pressure after birth can also require attention, particularly with headache or visual changes.

Practical steps

Follow your surgical team’s movement instructions and take short, gentle walks as allowed. Avoid sitting still for long periods, keep clothing loose around the groin and thighs, and raise your legs when resting if this is comfortable. Do not massage a painful or suddenly swollen leg until a clinician has assessed it.

Take prescribed medicines as directed and keep your wound-care review. Postnatal physiotherapy may help restore movement safely, but new or worsening swelling should be discussed before exercise is increased.

When to seek care

Get urgent medical help for chest pain, sudden breathlessness, coughing blood, faintness, or a fast heartbeat. Seek prompt assessment for swelling that is clearly greater on one side, painful, warm, red, or rapidly increasing. A severe headache, blurred vision, upper abdominal pain, or raised blood pressure after birth also needs urgent review because postpartum pre-eclampsia can occur.
