What should you know about a swollen big toe and gout?

Short answer
Gout commonly presents with sudden swelling, warmth, redness, and intense pain in the big-toe joint, but a hot swollen joint needs medical assessment because infection can look similar.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical pattern
- Sudden pain and swelling can focus on the big-toe joint.
- Avoid
- Tight footwear and self-changing prescribed treatment.
How gout affects the toe
Gout develops when urate crystals irritate a joint. The big toe is a frequent site, and an attack may start abruptly, with pain from light contact, warmth, and shiny or tight skin. The ankle or other joints can also become swollen or painful. Chronic kidney disease can affect how the body clears urate, while body weight and other health factors may influence risk; neither factor proves that a toe problem is gout. A clinician may use your history, examination, blood tests, or joint-fluid testing to confirm the cause.
During an attack
Rest the foot and raise it when possible. Keep pressure off the toe with roomy footwear or open footwear, and use a cool wrapped pack for comfort. Drink normally unless a clinician has restricted your fluids. Do not start, stop, or change prescribed urate-lowering medicine without advice; medicines used for a flare may not be suitable for everyone, especially with kidney disease or other conditions. Arrange a review to discuss treatment and ways to reduce future attacks.
When prompt care matters
Seek urgent care for fever, chills, feeling very unwell, a wound near the joint, or rapidly worsening redness and pain. These features can indicate infection, which needs prompt treatment. Contact a clinician if this is your first suspected attack, the pain is severe, or symptoms are not improving. Repeated episodes, persistent joint swelling, or new ankle swelling should be reviewed so the diagnosis and longer-term plan are clear.
Questions to ask
How will you distinguish gout from infection? Do I need joint-fluid testing or blood tests? Which pain medicines are safe with my other medicines or kidney health? Should I discuss a long-term urate-lowering plan?

Questions people ask
Is every swollen big toe gout?
No. Gout is a common explanation for a sudden hot, painful toe, but injury, infection, and other joint conditions can appear similar.
Can gout affect the ankle?
Yes. Gout may inflame the ankle or another joint, not only the big toe.
