Swollen ankles in the second trimester: what to know

Short answer
Swollen ankles in the second trimester can reflect normal fluid changes, but sudden or marked swelling needs maternity advice.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Often noticed
- Swelling that increases later in the day and improves with rest
- Tell your team
- Any sudden change or swelling affecting the face or hands
What it can mean
Pregnancy increases the amount of fluid in the body and the growing uterus can slow blood return from the legs. Swelling in both feet or ankles that builds later in the day and eases with rest is common. It should still be mentioned at antenatal appointments, particularly if it is new. Swelling by itself does not diagnose a pregnancy complication. A sudden change, swelling of the face or hands, severe headache, vision disturbance, pain under the ribs, or feeling very unwell can signal a problem needing prompt review.
Ways to ease it
Change position often, take gentle walks if your maternity team has said activity is suitable, and flex your feet while sitting. Rest on your side with your legs supported, wear comfortable shoes, and avoid socks that leave deep marks. Keep drinking normally and do not restrict fluids or take diuretics unless prescribed. Ask before using compression stockings so the size and strength are appropriate. Record when the swelling appears and whether one side is more affected.
When to contact maternity care
Contact your maternity unit promptly for sudden or rapidly increasing swelling, especially in the face or hands, or if it comes with a severe headache, visual symptoms, upper abdominal pain, vomiting, or feeling unwell. Seek urgent help for one-sided leg swelling with pain, warmth, or redness, or for breathlessness, chest pain, or fainting. Do not wait for a routine appointment when these symptoms occur.
Questions for your maternity team
Does this pattern fit expected pregnancy swelling? Should my blood pressure and urine be checked sooner? Are compression stockings suitable? What symptoms should make me call the maternity unit immediately?
