What the swelling may mean

Pregnancy increases body fluid, and labour, intravenous fluids, reduced movement, and hormonal changes can leave extra fluid in the feet and legs after birth. It may feel tight by the end of the day and improve when you rest with your legs raised. Both sides being affected in a similar way is generally less concerning than a new difference between legs.

A clot in a deep leg vein can also cause swelling, often with warmth, tenderness, redness, or a heavy feeling on one side. The symptom does not identify the cause by itself, so a clinician may need to examine your leg and decide whether testing is needed.

What you can do

Take short, gentle walks when you are able, avoid standing still for long periods, and rest with your legs supported. Wear comfortable footwear and avoid tight socks that leave deep marks. Drink normally and follow your maternity team’s advice, especially if you had high blood pressure or medical treatment during labour.

Do not massage a painful, hot, or suddenly swollen leg until a clinician has assessed it. Keep track of when the swelling began, whether one side is changing faster, and any associated pain or breathlessness.

When to seek care

Contact your maternity unit or a clinician promptly for swelling that is new, marked, painful, warm, red, or clearly worse on one side. Seek emergency help for chest pain, sudden shortness of breath, coughing blood, faintness, or severe difficulty breathing, because these can signal a clot that has travelled to the lungs.
