Understanding ongoing swelling

A clot can damage vein valves and make it harder for blood to return from the limb. Fluid may then collect around the ankle or lower leg, often becoming more noticeable after standing and easing with rest. This is sometimes called post-thrombotic change.

Swelling is not automatically proof that a new clot has formed. However, a change from your usual pattern matters, especially if one leg becomes more swollen, warm, red, tender, or heavy. Infection, injury, medication effects, and other circulation problems can look similar.

What to do safely

Take anticoagulant medicine exactly as prescribed and do not stop it without medical advice. Keep follow-up appointments, move gently as advised, and avoid sitting still for long periods. Elevating the leg during rest may reduce fluid build-up.

Ask the clinician managing your DVT whether a properly fitted compression garment is suitable; do not use one over an open wound or if circulation has not been checked. Do not massage a newly swollen or painful leg until a clinician has advised you.

When to seek care

Call emergency services for sudden breathlessness, chest pain, coughing blood, fainting, or a fast or irregular heartbeat. These symptoms can indicate that a clot has travelled to the lungs.

Contact your DVT team promptly for new or rapidly increasing swelling, new calf pain, warmth, redness, or bleeding while taking an anticoagulant. Facial swelling with breathing or throat symptoms is also an emergency and requires immediate help.
