What should you know about sulphur for perioral dermatitis?

Perioral dermatitis often causes small red bumps around the mouth, nose, or eyes, with dryness, scaling, or sensitivity. It can resemble pimples, but squeezing or layering acne products may make the irritation harder to settle. The skin around the lips may be spared, although patterns vary.

Sulphur is included in some topical preparations because it can reduce surface oil and scaling. It is not a universal treatment, and a product that feels helpful on one area may be too irritating near the mouth or eyes. Recent use of a steroid cream, cosmetics, toothpaste, or fragranced products is useful information for a clinician.

Use lukewarm water and a bland cleanser, then apply a fragrance-free moisturiser if needed. If a clinician recommends sulphur, follow the stated schedule, test a small area, and keep it away from the lips, nostrils, and eyelids. Avoid adding several new products at once.

Do not restart a topical corticosteroid on the face without medical guidance; stopping or changing it may need a planned approach. If sulphur is unsuitable, a clinician may consider topical antibiotic therapy or oral treatment depending on the extent and persistence of the rash.

Arrange medical review when the rash reaches the eyes, persists despite a simplified routine, or repeatedly returns. Seek prompt help for eye pain, vision changes, facial swelling, extensive blistering, or breathing difficulty after a product.
