What should you know about stiff neck lump?

The neck contains lymph nodes, the thyroid gland, muscles, salivary glands and other tissues. A recent cold, throat infection or dental problem can make a lymph node tender and enlarged. Muscle strain can cause stiffness and a tight area, while thyroid swelling may be felt low at the front of the neck. Less commonly, persistent or unusual findings can be associated with Head and Neck Cancer, Lymphoma or Thyroid Cancer. A neck lump does not by itself identify its cause.

A clinician may check its location, size, mobility, tenderness and duration, and examine the mouth, throat and thyroid. Depending on the findings, tests may include imaging or a sample of cells. Do not repeatedly press or massage the lump, as this can irritate the area and make changes harder to judge.

Arrange a medical assessment if the lump persists, grows, feels hard or is not clearly linked to a short-lived infection. Note when it appeared, whether it moves, and any fever, sore throat, voice change, night sweating, weight change or swallowing difficulty. Keep a list of medicines and recent infections. If the thyroid is involved, the clinician may discuss Thyroid Removal Surgery only after appropriate assessment.

Seek urgent help for difficulty breathing, inability to swallow saliva, rapidly increasing swelling, severe neck pain with fever, new weakness or confusion. Arrange prompt review for a persistent lump with unexplained weight loss, night sweats, coughing blood, voice change or trouble swallowing. These warning signs call for assessment; they do not establish a cancer diagnosis.

Ask what structures could cause the lump, what examination or scan is appropriate, how long to monitor it, which changes should prompt earlier review, and whether a sample is needed to distinguish infection, inflammation and cancer.
