What may be happening

When you stand, blood briefly shifts towards your legs. The body normally tightens blood vessels and adjusts the heart rate to keep blood moving to the brain. If that response is slow or insufficient, symptoms can appear after rising from a chair or bed, after a hot shower, or during prolonged standing. Dehydration, illness, some medicines, prolonged bed rest, and problems affecting the heart or nervous system can contribute. A clinician may compare blood pressure and pulse while you are lying or sitting and then standing, while also reviewing symptoms and medicines. Dizziness has many possible causes, so the pattern matters.

What you can do

Rise gradually: sit on the edge of the bed, move your feet, and stand while holding a stable surface. If symptoms begin, sit or lie down promptly and raise your legs if comfortable. Drink fluids regularly unless a clinician has told you to restrict them. Avoid standing still for long periods, and consider gentle leg movements when you must remain upright. Do not change blood-pressure medicine or add salt without medical advice. Keep a record of when symptoms occur, what you were doing, and any medicines taken, then arrange a GP consultation for recurring episodes.

When to seek care

Seek urgent help if fainting is accompanied by chest pain, severe breathlessness, a fast or irregular heartbeat, significant injury, confusion, or weakness on one side. Get prompt medical advice for repeated fainting, symptoms during exercise, black stools, heavy bleeding, or inability to keep fluids down. A single brief episode may still deserve assessment when it is new, unexplained, or happening more often. Until the cause is clearer, avoid driving or working at heights if you feel close to fainting.

Ask whether your medicines, hydration, heart rhythm, blood count, or nervous system could be contributing; whether you need lying and standing measurements; and what symptoms should trigger urgent help. Tell the clinician about falls, palpitations, bleeding, vomiting, or recent illness.
