Stage IV lung cancer: what to know

Stage IV lung cancer means cancer is present in the other lung, in distant organs, or in fluid around the lung or heart, depending on the pattern found on scans and tests. The exact subtype matters because small-cell and non-small-cell lung cancers are treated differently. Genomic tumour profiling may look for changes that can guide targeted treatment.

Possible symptoms include cough, breathlessness, chest discomfort, tiredness, weight loss, or a hoarse voice. A hoarse voice can have many causes and does not by itself diagnose lung cancer. If cancer affects the brain, symptoms may include headaches, seizures, weakness, confusion, or changes in balance.

Your team may discuss chemotherapy, targeted treatment, immunotherapy, radiotherapy, or a combination. Surgery such as a lung lobectomy is considered only in selected situations, based on where the disease is and whether removing part of the lung would help. Palliative care can begin alongside cancer treatment to support breathing, pain, energy, appetite, sleep, and emotional wellbeing.

Bring scan reports and medicine lists to appointments, ask how treatment will be monitored, and tell the team about smoking, nutrition, work, and family needs. A written plan should include who to call for fever, worsening breathlessness, or treatment side effects.

Get urgent help for severe or rapidly worsening breathlessness, coughing up a large amount of blood, new confusion, a seizure, sudden weakness, chest pain, fainting, or blue lips. During treatment, contact the oncology team promptly about fever, shaking chills, or an inability to keep fluids down.

Ask which lung cancer subtype is present, whether genomic profiling is appropriate, where the cancer has spread, what each treatment is intended to do, how side effects will be managed, and whether palliative care or a clinical trial could support your goals.
