Spotty bleeding after menopause: what to know

After menopause, bleeding may come from delicate, dry vaginal or vulval tissue linked with genitourinary syndrome of menopause. Cervical or vaginal inflammation, a polyp, or thickening of the womb lining can also cause spotting. Womb cancer, cervical cancer, and vaginal cancer may present with postmenopausal bleeding, so the symptom deserves an examination rather than guesswork. Most causes are not cancer, but only an assessment can sort them apart.

Arrange an appointment with a gynaecology or primary-care clinician and record when the spotting happened, its colour, amount, and whether it followed sex. Mention hormone therapy, vaginal products, blood-thinning medicines, and any pelvic symptoms. Assessment may include an examination and, depending on the findings, imaging or a sample from the womb lining.

Seek urgent medical help for heavy or ongoing bleeding, faintness, marked weakness, severe pelvic pain, fever, or bleeding with a possible pregnancy. Otherwise, book a prompt routine assessment for any bleeding after menopause, including a single pink or brown spot.

Ask what area may be bleeding, whether your medicines or tissue dryness could contribute, and which examination or test is appropriate. Ask when to return if spotting recurs.
