Spotting After Sex During Perimenopause

During perimenopause, changing hormone levels can make periods irregular and the vaginal tissues drier or more sensitive. Friction during sex may then cause minor surface bleeding. Spotting can also come from a cervical problem, infection, a polyp, or another condition, so perimenopause should not be assumed to explain every episode.

Tell a clinician if you have heavy periods, bleeding between periods, bleeding after a long gap without periods, pelvic pain, unusual discharge, or bleeding that keeps returning. Your cycle history and other symptoms, such as hot flushes or night sweats, help provide context but do not replace an examination.

Write down when the spotting occurs, how long it lasts, and whether sex was painful or the vagina felt dry. A water-based lubricant may reduce friction. Arrange a review for repeated post-sex bleeding, any bleeding that concerns you, or symptoms after periods have stopped. A clinician may examine the vagina and cervix, check for infection, review cervical screening, and discuss treatment for dryness or other causes.

Seek urgent care for heavy bleeding, severe pelvic pain, faintness, dizziness, fever, or feeling very unwell. Prompt assessment is also appropriate for bleeding after a prolonged period without menstruation. Keep a record of the bleeding to share with the clinician.
