Spotting After Sex and Endometriosis

Endometriosis involves tissue similar to the womb lining growing outside the womb. It can be linked with pelvic pain, painful periods, pain during sex, or trouble getting pregnant. Spotting after sex is not specific to endometriosis, so it should not be used to confirm or rule out the condition.

Bleeding may instead reflect friction, vaginal dryness, a cervical change, or an infection. The pattern matters: pain deep during sex, worsening period pain, persistent pelvic pain, bowel or bladder symptoms around periods, and difficulty conceiving can provide useful context for a clinician.

Keep a symptom diary covering sex, spotting, pain, periods, bowel or bladder symptoms, and any treatments. Book a gynaecology appointment when bleeding repeats, pain is affecting intimacy, or symptoms interfere with daily life. Assessment may include an examination, infection testing, imaging, and discussion of hormonal treatment or other options. Treatments such as the combined pill or hormonal coil may be considered for endometriosis symptoms; the choice depends on your health and pregnancy plans.

Get urgent help for heavy bleeding, severe or rapidly worsening pelvic pain, faintness, fever, vomiting, or marked weakness. Seek prompt advice if you may be pregnant and have bleeding with pain. These features need assessment even when endometriosis is already diagnosed.
