What should you know about a small lump at the front of the neck?

This area contains the thyroid gland, muscles, soft tissues, and lymph nodes. A lump may be related to thyroid enlargement, a fluid-filled or solid thyroid nodule, or a lymph node responding to irritation or infection nearby. A front-neck lump can also arise from other benign tissue changes. Thyroid cancer presents with a neck lump in some people, while head and neck cancer or lymphoma can involve nodes in the neck. These conditions cannot be distinguished by touch alone.

Notice whether the swelling rises when you swallow, whether it is tender, and whether it has changed in size. Record when you first felt it and any accompanying hoarseness, throat discomfort, cough, or swallowing change.

Arrange an appointment with a doctor, especially if the lump remains after a recent illness or continues to change. The clinician may examine your mouth and throat, feel the thyroid and lymph nodes, and check your voice and swallowing. Depending on those findings, assessment may include a neck ultrasound, blood tests for thyroid function, or a sample taken with a fine needle. Do not squeeze or repeatedly press the area, because this can make tenderness harder to interpret. Bring a list of medicines and a note of related symptoms.

Seek urgent medical help if the swelling is accompanied by difficulty breathing, inability to swallow liquids, rapidly worsening pressure in the throat, or sudden voice change. Contact a doctor soon for a lump that is firm, fixed, enlarging, or associated with persistent hoarseness, unexplained weight loss, night sweats, or enlarged nodes elsewhere. Urgency is based on these warning features, not on the fact that a lump is small.

Ask which structure seems to be causing the lump, whether it moves with swallowing, and what examination or imaging is appropriate. You can also ask when results will be available, whether a needle sample is needed, and what change should prompt an earlier review.
