What should you know about small hard lump on neck under skin?

Neck lumps can follow a throat, dental or skin infection, and reactive lymph nodes may be tender. Other possibilities include a cyst, an inflamed skin structure, a thyroid nodule or a less common tumour. Head and neck cancer, lymphoma and thyroid cancer can present with neck swelling, but a lump does not prove any of these conditions. Its depth, mobility, tenderness, growth and relationship to swallowing help guide assessment.

A clinician may examine the mouth, throat, thyroid and lymph-node areas, then arrange ultrasound or other imaging. A needle sample may be recommended when the cause is not clear or the lump has concerning features.

Arrange a medical review if the lump remains, enlarges, feels fixed or is not linked to a short-lived infection. Record when you first noticed it, whether it changes with swallowing, and whether you have pain, fever, dental symptoms, voice change or difficulty swallowing. Do not squeeze or repeatedly press it. The doctor may refer you to an ear, nose and throat team or another specialist; thyroid removal surgery is considered only when findings support a thyroid-related reason.

Seek emergency care for trouble breathing, inability to swallow saliva, rapidly increasing swelling, severe neck pain after an injury, or new weakness or confusion. Arrange prompt review for a persistent enlarging lump, unexplained hoarse voice, coughing or swallowing difficulty, night sweats, or unexplained weight change.
