Small Bald Spot on Top of Head: What to Know

A smooth, clearly defined patch without scale can occur with alopecia areata. This condition presents with areas where hair sheds from an otherwise relatively normal-looking scalp. A patch with redness, scaling, tenderness, broken hairs, or swollen glands can fit scalp ringworm, which needs medical treatment. Tight hairstyles, repeated rubbing, chemical processing, or scratching may also break hairs in one area.

Look at whether the patch is enlarging, whether short broken hairs remain, and whether the skin feels itchy or sore. A photograph taken in the same lighting can help show change over time.

Handle the area gently: pause tight styles and harsh chemical treatments, use a mild shampoo, and avoid picking or vigorous brushing. Do not share combs, hats, pillows, or towels if there is scaling or concern about infection. Arrange a clinician or dermatologist assessment if the patch persists, expands, or returns. Hair restoration is considered only after the cause is identified and hair loss has stabilised.

Seek prompt medical care for rapid spread, marked pain, pus, crusting, fever, or a swollen tender scalp. Arrange an earlier review if eyelashes or eyebrows are also thinning, or if the scalp is shiny and scar-like. These features can change the evaluation and treatment plan.
