A sinus-like headache without congestion may come from migraine, tension, dehydration, heat exposure or another cause rather than a sinus infection. Pain over the brow, cheeks or behind the eyes can feel as though it is coming from the sinuses, but location alone cannot identify the cause. Notice whether light, sound, movement, nausea, jaw clenching, long screen use, missed meals or hot weather changes the pattern.

Migraine can cause pressure or throbbing in the forehead or around one eye, with nausea or sensitivity to light and sound.
