Sharp stabbing pain during ovulation

Ovulation pain, also called mittelschmerz, may occur when an ovary releases an egg. It can feel like a twinge, cramp, or stab on one side of the lower abdomen and may settle without treatment. The same timing can overlap with other pelvic problems. Ovarian cysts can cause pain, pressure, or sudden worsening if a cyst changes. Endometriosis can also present with pelvic pain and may follow a repeating cycle pattern. Timing alone cannot confirm ovulation as the cause.

Rest, use gentle warmth, and record the date, side, duration, and intensity. Note bleeding, discharge, fever, nausea, bowel changes, urinary symptoms, and whether pregnancy is possible. If the pain is mild and settles, mention recurring episodes at a routine appointment. Seek advice sooner when the pattern changes, lasts longer than usual, or interferes with daily activities.

Seek urgent care for sudden severe pain, faintness, heavy bleeding, fever, repeated vomiting, marked abdominal swelling, or possible pregnancy. Prompt review is needed for pain that persists, returns with increasing intensity, or is accompanied by unusual discharge or pain when passing urine. These features may point away from uncomplicated ovulation pain.

Ask whether your cycle pattern supports ovulation pain and whether a pregnancy test, pelvic examination, or ultrasound is appropriate. Discuss contraception, fertility goals, prior cysts, and pain between periods. Ask how to recognise a change that requires urgent review.
