Severe dry, itchy scalp

Dandruff and seborrheic dermatitis can produce flakes with itch, while scalp psoriasis may create thicker, sharply defined scale. Head lice can cause intense itch and visible nits attached to hairs. Folliculitis is more suggestive when tender or pus-filled bumps surround hairs. A reaction to dye, shampoo, oils, or styling products can mimic these conditions.

Use lukewarm water and a gentle shampoo, rinse thoroughly, and avoid scratching with nails. Stop any newly introduced hair product if symptoms began after its use. A medicated shampoo treatment may be useful for dandruff or seborrheic dermatitis when used as directed. Wash combs and avoid sharing them if lice are possible.

See a clinician if severe itch continues, flakes return despite appropriate shampoo use, or hair loss develops in patches. Get prompt care for spreading redness, warmth, swelling, fever, painful bumps, drainage, or sores. If live insects or firmly attached nits are seen, ask about head lice treatment.
