What this injury involves
Sesamoids sit under the front of the foot and help the big toe bend while protecting tendons. Repeated running, jumping, dancing, a change in training, or footwear pressure can overload them. Pain is usually focused beneath the big toe and may worsen when pushing off; swelling, tenderness, and a limp can follow. A clinician may compare both feet and use imaging, although an early stress injury may need more than one assessment. A stress fracture is different from ordinary foot pain, so continuing impact activity can delay healing.
Protecting the foot
Stop running, jumping, and painful push-off until assessed. Wear a stiff-soled shoe or the support recommended by your clinician, and avoid barefoot walking on hard surfaces. Ice wrapped in cloth can ease soreness. Do not return to impact exercise simply because pain has settled for a day; bone needs a gradual loading plan. Your clinician may review training, foot mechanics, nutrition, and bone health, including vitamin D when appropriate. Vitamin D supplementation should be taken only in the dose advised for your situation.
When to seek care
Arrange prompt assessment for persistent pain under the big toe, pain that changes your walking, or tenderness after a training increase. Seek urgent care after major injury, with severe swelling or deformity, inability to bear weight, a cold or numb foot, or rapidly worsening pain. These signs may indicate a different or more extensive injury.
