What belongs in a seizure log

Write the date and time, where the person was, what they were doing, sleep quality, food and fluids, illness, fever, stress, alcohol or substance exposure, and any missed or changed medicine. Describe the first sign, awareness, speech, eye position, movements, skin colour, breathing, injury, and behaviour afterwards. Include how long the active event seemed to last and how long recovery took. A witness may notice details the person cannot recall. Patterns can guide assessment, but a diary cannot prove a cause. How to keep it useful

Use the same simple format each time and separate observed facts from guesses. Record medicine doses as prescribed and note vomiting or difficulty keeping them down. Ask a trusted witness to contribute, and only record a video when doing so does not delay first aid or compromise privacy. Bring the log to appointments. Never change or stop anti-seizure medicine because an entry looks reassuring; discuss patterns with the prescriber.

When the log should not delay help

Call emergency services for a prolonged seizure, repeated seizures without normal recovery, serious injury, breathing difficulty, or a seizure in water. Seek urgent care for a first suspected seizure, fever with neck stiffness, pregnancy, diabetes, or ongoing confusion, weakness, or severe headache. Record the emergency details afterwards if it is safe, but focus first on protecting the person and getting help.
