Second Period After Miscarriage: What to Know

Bleeding immediately after a miscarriage is not a menstrual period. Your next period begins after ovulation resumes, so the timing can vary. Pregnancy can also occur before that period, including if bleeding has not yet returned to your usual pattern.

The second cycle may not match your pre-pregnancy pattern. You might notice different cramping, breast tenderness, mood changes, clotting, or flow. These changes can reflect the time your hormone levels and uterine lining need to return to their usual rhythm.

Bleeding between periods, ongoing pelvic pain, fever, or an unpleasant vaginal odour is not something to dismiss as a normal cycle change. These symptoms can need assessment for retained pregnancy tissue or infection. A clinician may recommend a pregnancy test, examination, blood tests, or a transvaginal ultrasound to clarify what is happening.

Record the first day of each bleed, how quickly pads become wet, the presence of clots, pain intensity, temperature, and any unusual discharge. This gives your clinician a clearer picture than trying to remember the pattern later.

Use sanitary pads if you need to monitor flow closely, and follow the discharge instructions you received after the miscarriage. Ask a pharmacist or clinician which pain medicine is suitable for you, particularly if you may be pregnant again or have another medical condition. If you do not want another pregnancy yet, discuss contraception because ovulation may happen before the next period.

If your cycle remains noticeably irregular, a home pregnancy test is positive, or your period does not return as expected, arrange a review. Depending on the findings, care may include observation, Medical Management of Miscarriage, or Surgical Management of Miscarriage.

Seek urgent medical help for very heavy bleeding, fainting, severe or worsening abdominal or pelvic pain, shoulder-tip pain, marked weakness, or difficulty breathing. These symptoms can signal significant blood loss or another urgent problem.

Contact a healthcare professional promptly if you have a fever, chills, increasing tenderness, foul-smelling discharge, persistent bleeding, or a positive pregnancy test with pain or bleeding. Do not wait for a second period to finish before asking for advice.

Ask whether your bleeding pattern fits recovery, whether you need an examination or Transvaginal Ultrasound, and when it is appropriate to try for pregnancy again. If you had medication or a procedure during the miscarriage, ask whether that changes what to expect from your next cycles.
