What should you know about second degree tear stitches?

A second-degree tear extends beyond the skin into the muscle of the perineum, the area between the vagina and anus. Dissolvable surgical sutures are commonly used, so you may not need them removed. Tenderness, pulling, bruising, or discomfort when sitting can occur while the tissue settles. Passing urine may sting if urine touches the wound.

Keep the area clean with plain water, pat it dry, and change sanitary pads regularly. Wash your hands before and after touching the area. A wrapped cold pack can ease swelling for short periods. Rest, avoid straining, and use any prescribed pain relief as directed. Prevent constipation with fluids and fibre, and ask whether a stool softener is suitable. Follow the wound-care advice given after delivery and attend any planned review.

Contact your maternity team promptly if pain or swelling is getting worse, the wound opens, bleeding becomes heavy, or the area develops pus, an unpleasant smell, or spreading redness. Fever, feeling very unwell, or difficulty controlling urine or stool also needs assessment. Seek urgent help for severe bleeding, faintness, or severe pain. Do not wait for a routine appointment if symptoms are rapidly worsening.

Ask how the tear is healing, which pain relief and bowel-care options are appropriate, and when you can resume exercise, sex, driving, or lifting. Ask who to contact if the stitches loosen or symptoms return.
