Scarring Due to Hair Loss: What to Know

Scarring hair loss can leave skin smooth or shiny, with fewer visible follicle openings. It may be accompanied by itch, burning, tenderness, redness, scale, or small crusts. Hair loss itself has many possible causes: alopecia areata can form sharply defined patches, while male or female pattern hair loss often causes gradual thinning. Scalp ringworm may cause scale and broken hairs; telogen effluvium commonly causes diffuse shedding; repeated pulling can cause traction alopecia.

Photograph the affected area in consistent lighting and note changes in symptoms, hairstyles, illnesses, new medicines, and chemical treatments. Handle the hair gently and pause tight styles, extensions, bleaching, or heat that worsen tenderness. A dermatologist may use magnification, review the pattern, and take a small biopsy when the diagnosis is uncertain. Treatment depends on the cause; finasteride treatment is relevant to selected pattern hair-loss situations, not every form of hair loss.

Book an assessment if a patch is enlarging, the scalp feels painful or inflamed, or the skin appears shiny and follicle openings are disappearing. Seek urgent care for fever, pus, spreading warmth, marked swelling, or severe pain. Do not wait for extensive loss before evaluation, because established scar tissue cannot be reversed by treating the original inflammation.

Ask whether the loss is scarring, whether a biopsy is appropriate, what may be driving inflammation, and how you will know if treatment is controlling progression.
