Sagging belly after a C-section: what to know

Short answer
A sagging belly after a C-section can reflect normal postpartum changes, healing, stretched skin and weakened abdominal muscles; recovery is gradual.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common contributors
- Stretched skin, healing tissues, swelling and abdominal muscle changes
- Helpful next step
- Gentle movement followed by personalised rehabilitation when cleared
- Do not ignore
- Wound changes, fever, severe pain or sudden headache and vision symptoms
What the change may mean
Pregnancy stretches the abdominal wall and skin, while the uterus takes time to reduce after birth. Swelling, fluid shifts, reduced activity and a fold of skin above the scar can make the lower abdomen look or feel heavier. Some people also have diastasis recti, a gap between the abdominal muscles that can cause a domed shape when sitting up. The appearance alone cannot show whether healing is complete.
Supporting recovery
Follow your caesarean wound instructions and increase walking gently as comfort allows. Avoid lifting beyond the limits given by your maternity team and stop an activity that increases pain, pulling or bleeding. Once cleared, gradual pelvic-floor and deep-core work may improve strength and movement; [Postnatal Physiotherapy](/treatments/pelvic-floor-and-rehabilitation/postnatal-physiotherapy/) can provide an individual assessment. Nourishing meals, fluids and rest support recovery, but creams, binders or exercises cannot instantly remove loose skin.
When to seek care
Contact your maternity team promptly for a wound that becomes more red, hot, swollen or painful, opens, or drains pus; fever; heavy bleeding; worsening abdominal pain; breathlessness; chest pain; or a painful swollen leg. Seek urgent help for severe headache, vision changes, sudden swelling or upper-abdominal pain, which can signal [Pre-eclampsia](/conditions/pre-eclampsia/). A persistent bulge, pelvic heaviness or loss of bladder or bowel control also deserves review.
Questions for your doctor
Ask whether your scar and abdominal wall are healing as expected, whether diastasis recti is present, when core exercise is appropriate, and which symptoms should trigger an examination. You can also ask whether [Caesarean Section](/treatments/pregnancy-and-birth-care/caesarean-section/) recovery advice applies to your situation.

Questions people ask
Will a sagging belly disappear after a C-section?
The shape may change as swelling settles, muscles strengthen and tissues heal, but loose skin or a lasting abdominal gap may remain. A clinician or physiotherapist can assess persistent concerns.
Can I exercise to tighten my abdomen?
Begin with gentle movement and follow clearance from your maternity team. Progress to deep-core and pelvic-floor exercises gradually, especially if pain, pressure or a bulge occurs.
