What it means

The sacrum sits at the base of the spine between the hip bones. Repeated running, jumping, a sudden training increase, reduced bone strength or altered walking mechanics can load it beyond its ability to remodel. Pain is often deep in the buttock, low back or pelvis and may increase with walking, stairs or single-leg loading. It can be mistaken for a muscle or joint problem. A clinician may examine your gait and tenderness and choose imaging; early stress injuries are not always visible on an initial plain X-ray. A broader stress-fracture assessment may also consider vitamin D deficiency.

What to do next

Stop the activity that provokes pain and arrange a clinical assessment, especially if pain persists at rest or changes your walking. Do not test recovery with running or jumping. Your clinician may recommend relative rest, a walking aid or other protected loading, followed by gradual return as symptoms settle. Physiotherapy can address hip and trunk strength, balance and the training or movement pattern that contributed. Ask whether bone-health testing or vitamin D supplementation is appropriate rather than starting supplements on your own.

When to seek care

Seek urgent care for new leg weakness, numbness around the groin, loss of bladder or bowel control, fever with severe back or pelvic pain, or inability to bear weight. Prompt assessment is also sensible after a fall, when pain is rapidly worsening, or when night pain is persistent. If you are limping, stop impact activity until a clinician has assessed the cause; limping can increase load on other areas such as the foot, heel, shin or thigh.
