How it presents

Hamstrings cross the back of the hip and knee and work hard during sprinting, jumping, kicking, and sudden braking. A strain may cause a sudden snap or pain, followed by tenderness, tightness, swelling, bruising, or a shortened stride. Thigh Pain can describe the area, but pain in the back of the thigh may also have other sources. Examination considers the injury mechanism, exact tenderness, strength, flexibility, and walking pattern.

The amount of pain does not reliably show the full extent of an injury. Note whether sitting, bending the knee, lengthening the leg, or faster walking changes symptoms. Repeatedly stretching hard or sprint-testing the leg can irritate healing tissue.

Managing the injury

Stop sprinting and other painful activity at first, and walk only as comfortably as possible. Support the leg when resting and avoid forceful massage or stretching soon after an acute injury. A clinician can advise on symptom relief and the right time to begin loading. Exercise Therapy usually progresses from comfortable activation to strengthening through larger ranges, then faster movements. Sports Physiotherapy can address running technique and sport demands. A Return-to-Sport Programme uses staged tests rather than a single pain check. PRP Injection is sometimes discussed for selected soft-tissue injuries, but suitability requires an individual assessment.

When to seek care

Seek assessment when pain is sudden or substantial, walking is markedly limited, bruising spreads, swelling increases, or you felt a pop. Get urgent help after major trauma, with severe weakness, numbness, a cold or unusually pale foot, or inability to bear weight. Arrange review for recurrent strains or symptoms that fail to improve, since rehabilitation needs can differ between muscle injuries.
