What should you know about refractory hypertension?

Hypertension is called refractory when blood pressure stays above the person's agreed target despite a carefully managed treatment plan. Before using this description, clinicians confirm the readings with correct cuff size, repeated measurements, or home monitoring. They also check missed doses, interactions, pain, stress, stimulants, and medicines that can raise blood pressure.

The review may look for conditions linked with difficult-to-control pressure, including kidney disease, obesity, obstructive sleep apnoea, or hormone problems. High blood pressure may have no symptoms, but chest pain, severe headache, weakness, speech trouble, or breathlessness can signal an emergency. A reading by itself does not explain the cause.

Book a review and bring a list of every medicine, supplement, and substance you use, plus home readings and the time each was taken. Take prescribed treatment consistently and do not double a missed dose unless instructed. Your clinician may check technique, adjust medicines, request blood or urine tests, and assess the heart, kidneys, sleep, and other risks.

Reduce added salt, avoid tobacco, keep alcohol within your clinician's advice, and build activity gradually if safe. Do not rely on symptoms to judge control; hypertension often causes none.

Call emergency services for chest pain, severe breathlessness, fainting, new weakness or numbness on one side, difficulty speaking, sudden vision change, or a severe unusual headache. These symptoms can occur with heart attack or stroke and need immediate assessment. If a very high home reading is unexpected but you feel well, repeat it using correct technique and contact your clinical team promptly for advice rather than changing medicine yourself.
