What should you know about refractory heart failure?

Heart failure means the heart is not pumping or filling well enough for the body's needs. Refractory disease is difficult to control: breathlessness, swelling, fatigue, cough, or reduced ability to function continues or worsens despite the treatment plan. Heart rhythm problems such as atrial fibrillation, high blood pressure, kidney disease, infection, or missed doses can aggravate symptoms and should be checked.

Symptoms can change gradually or suddenly. Daily weight, swelling, breathing, blood pressure, pulse, and medicine effects give the care team useful information, but home measurements do not replace an examination. A chronic disease review can help coordinate medicines, tests, lifestyle advice, and follow-up.

Contact your heart-failure or GP team for a review if symptoms are not settling or your usual activity has become harder. Bring all medicines, including non-prescription products, and report dizziness, palpitations, reduced urine, new swelling, or changes in breathing. Take prescribed medicines as directed and do not alter water pills or restrict fluids unless your clinician gives a specific plan.

Follow the agreed advice about salt, fluids, activity, vaccinations, and monitoring. Record symptoms and readings at the same time each day when your team has asked you to do so. Ask whom to call outside clinic hours.

Call emergency services for severe breathlessness at rest, inability to lie flat because of breathing, chest pressure, fainting, new confusion, blue lips, or sudden weakness. Seek urgent advice for a rapid increase in swelling, a major change in heartbeat, or breathlessness that is quickly worsening. Sit upright while waiting for help and take your medicine list with you.
