Short answer

A really heavy period after pregnancy deserves assessment because cycle changes can be temporary, but bleeding may also signal anaemia or a treatable uterine problem.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Flow pattern, clots, pain and symptoms of anaemia
Possible consequence
Iron deficiency anaemia from ongoing blood loss

Why bleeding may change

Periods can be different when ovulation and hormones are settling after pregnancy, particularly if breastfeeding or if cycles have only recently returned. Heavy bleeding can also occur with uterine fibroids, adenomyosis, endometrial polyps or endometrial hyperplasia. Polycystic Ovary Syndrome and Perimenopause can disrupt ovulation and make bleeding less predictable. Heavy blood loss may lead to Iron Deficiency Anaemia, which can cause tiredness, breathlessness, dizziness, headaches or a racing heartbeat. A heavy period is a symptom, not a diagnosis, so the pattern and timing matter.

What to do next

Keep a simple record of bleeding days, clots, flooding, pain, dizziness and any bleeding between periods. Arrange a medical review if the change is marked, continues, or affects work, sleep or caring for your baby. Your clinician may check for anaemia, review medicines and contraception, perform an examination, and arrange imaging or other tests when indicated. Treatment depends on the cause and whether you are breastfeeding or trying to conceive. Do not start iron or hormone treatment without advice about the right option for you.

When to seek urgent help

Seek urgent care if bleeding is soaking through protection repeatedly, you feel faint or confused, have severe abdominal pain, chest pain, shortness of breath, or a very fast heartbeat. If pregnancy is possible, heavy bleeding with one-sided pain, shoulder-tip pain or collapse needs emergency assessment. These warning signs can reflect significant blood loss or a pregnancy-related problem and should not wait for a routine appointment.

Questions for your doctor

Ask whether your bleeding pattern suggests a problem with ovulation, the uterine lining or fibroids, whether you need a blood count and iron assessment, and which treatments are compatible with breastfeeding. Ask when imaging is useful and what symptoms should send you to urgent care.

Questions people ask

It can have several causes, many treatable; bleeding severity and red-flag symptoms guide how quickly to seek care.
