What should you know about really dark urine even when hydrated?

Short answer
Really dark urine despite normal hydration should be reviewed for medicines, pigments, liver-related changes or muscle injury.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Persistent colour
- Dark urine after normal drinking is worth checking if it continues.
- Bring details
- List medicines, supplements, exercise, heat exposure and associated symptoms.
- Urgent signs
- Very low urine output, confusion, severe weakness or severe pain need urgent help.
Why hydration may not change it
If you are drinking normally and urine remains very dark, concentration from dehydration is less likely but not ruled out; fluid needs vary with heat, sweating, illness and medicines. Orange, brown or tea-coloured urine may reflect a dye from a medicine, supplement or food. Bile pigments can darken urine when liver or bile-flow problems are present, especially with yellow eyes, pale stools or itching. Muscle injury after strenuous activity is another possibility, particularly with muscle pain or weakness. Dark urine does not by itself diagnose Hepatitis A or Hepatitis B.
What to do next
Keep drinking at your usual safe level rather than forcing extra water. Check recent medicines and supplements, note exercise and record whether the colour changes during the day. Do not discontinue prescribed treatment without professional advice. Book a clinical review if the colour persists, recurs, or comes with other symptoms. A clinician may examine you and request urine or blood tests.
When to seek care
Seek urgent care for very little urine, confusion, fainting, severe weakness, severe pain, repeated vomiting, or dark urine with muscle pain after heavy exercise. Arrange prompt review for yellow skin or eyes, pale stools, swelling, easy bruising or ongoing dark urine. If heat exposure preceded the change and you also feel dizzy, weak or unwell, review Heat Exhaustion information and seek help promptly.
Questions to take along
Ask whether the colour could come from a medicine or supplement, whether liver or muscle tests are needed, and what urine output or associated symptom should change the urgency of care.

Questions people ask
Does hydrated dark urine mean something serious?
Not necessarily, but persistence despite normal drinking makes medicines, pigments and other causes worth reviewing.
Should I drink much more water?
Avoid forcing large amounts quickly; drink at your usual safe level unless a clinician has given different advice.
