What is the purpose of immunotherapy?

Cancer cells can avoid immune defences or send signals that weaken an immune response. Immunotherapy changes that interaction. Some medicines remove a checkpoint that was holding immune cells back; others attach to cancer cells or help immune cells target them. It may be used before surgery, after another treatment, alongside other treatment, or when cancer has returned or spread. The aim may be to shrink visible cancer, slow its growth, reduce the chance of recurrence, or keep disease controlled. The plan is individual: a treatment that helps one cancer type may not suit another, and testing may show whether the tumour has features that make a particular medicine reasonable.

Bring a list of medicines, supplements, allergies and previous immune-related illnesses to the oncology appointment. Tell the team about autoimmune disease, organ transplant, pregnancy or plans for pregnancy, because these details can affect treatment choices. Ask how the medicine will be given, how often visits occur, what monitoring is planned and when results will be reviewed. Keep a written record of new symptoms rather than waiting for the next appointment. Do not start a supplement or stop a prescribed medicine without checking with the treatment team.

Immune effects can involve the skin, bowel, lungs, liver, hormone glands or other organs. Contact the oncology team promptly for new or worsening breathlessness, chest pain, severe diarrhoea, blood in stool, intense abdominal pain, yellowing of the eyes, marked weakness, confusion, a widespread rash, or a high temperature. Severe breathing difficulty, collapse or sudden confusion needs emergency care. Early assessment helps the team decide whether to pause treatment and provide treatment for inflammation or another cause.
