Pulled Hamstring Not Healing: What to Do

How the structures involved in Hamstring Strain differ from normal

Short answer

A pulled hamstring that is not healing needs a review of the diagnosis, activity load and rehabilitation plan.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Why recovery may stall

Ongoing pain can follow returning to sprinting, stretching or sport before strength and control have returned. Repeated small strains, a larger tear, irritation near the sitting bone or weakness in the hip and trunk can also keep the back of the thigh working harder. Sometimes the original problem was not a hamstring injury, especially when pain is vague, travels down the leg or comes from the back.

A clinician can review the injury mechanism, tenderness, walking pattern, knee and hip movement, and strength. Imaging is considered when the examination suggests a significant tear or another condition. Persistent thigh pain should not be judged only by how it feels on one day.

Steps that support a review

Pause the activity that repeatedly provokes symptoms, but avoid complete inactivity unless advised. Keep a simple record of walking tolerance, exercises and next-day soreness. A physiotherapist may adjust the load from gentle isometric work to controlled strengthening, then running and direction changes. Exercise therapy gives this progression structure.

Sports physiotherapy can identify technique, flexibility and strength differences that add load to the hamstring. A return-to-sport programme can set clear tests before competition. For selected cases, a specialist may discuss PRP injection alongside rehabilitation; it does not replace a careful diagnosis or progressive loading.

When to seek care

Book an assessment if pain is not trending better, keeps returning, or prevents normal walking, sleep or work. Seek prompt care after a new pop, sudden weakness, spreading bruising or a noticeable dip in the muscle. Urgent evaluation is needed for severe escalating pain, numbness, a cold foot or inability to bear weight.

Questions for your doctor

Ask whether the diagnosis still fits, whether imaging would change management, which exercise level is appropriate, and what improvement should look like over the next stage. Ask when a sports or rehabilitation review would be useful.

Questions people ask

Why does my hamstring hurt again after exercise?

The load may have advanced faster than strength and control, or the original diagnosis may need review.

Is persistent hamstring pain always a tear?

No. Persistent pain can have several causes, including referred pain, so an examination is useful.
