Pulled Back Shoulder Muscle

Short answer
A pulled back shoulder muscle often improves with relative rest, supported movement, suitable pain relief and gradual strengthening.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Protect initially
- Reduce overhead loading and heavy carrying
- Keep mobile
- Use gentle shoulder movement within comfort
- Urgent symptoms
- Chest symptoms, breathlessness or a cold, pale hand
Where the pain may come from
The muscles between the shoulder blade, spine and upper arm can be strained by lifting, reaching, throwing or prolonged posture. Pain may increase when you pull, reach overhead or move the shoulder blade. Shoulder joints, the neck and nerves can create similar pain, so weakness, tingling or restricted motion may change the assessment.
Helpful care
Temporarily reduce overhead work, heavy carrying and the motion that caused the pain. Keep the elbow, wrist and shoulder moving gently within comfort so the area does not become guarded. Cold wrapped in a towel may soothe a recent injury; warmth may help muscle tightness. Support the arm when resting, but avoid keeping it completely still for long periods. As pain settles, practise easy shoulder-blade movements and light resistance only when advised or tolerated. A physiotherapist can check shoulder and neck movement and build a graded programme.
When to get checked
Arrange an examination after a fall, visible deformity, substantial swelling, marked bruising, persistent weakness or inability to raise the arm. Seek urgent help for chest pressure, breathlessness, sweating or pain spreading to the jaw or arm. New numbness or a cold, pale hand also needs prompt care.
Questions to bring
Ask whether the pain is coming from muscle, shoulder joint or neck, which movements are safe, and when strengthening can start. Explain any clicking, night pain, weakness or tingling and identify the activity that triggered symptoms.

Questions people ask
Should I keep my shoulder completely still?
Usually not for long periods. Gentle, comfortable movement can limit stiffness while the painful load is reduced.
Could neck pain feel like a pulled shoulder muscle?
Yes. Neck joints or nerves can refer pain toward the shoulder, especially when tingling or weakness is present.
