What these lesions can be

Psoriasis lesions commonly appear as areas of inflamed, thickened, or scaly skin. They can itch, feel sore, crack, or bleed after scratching. The scalp, elbows, knees, hands, feet, and skin folds may be involved, though lesions can occur elsewhere. Nail pitting, thickening, or separation may provide another clue to psoriasis.

A skin lesion does not show how active the joint disease is. Some people have noticeable skin symptoms with little joint discomfort, while others develop joint pain or swelling when skin changes are mild or not visible. A clinician can distinguish psoriasis from eczema, infection, allergy, or another rash by examining it and asking about its course.

Caring for irritated skin

Use a plain, fragrance-free moisturiser after washing and whenever the skin feels dry. Bathe with lukewarm water, pat rather than rub, and choose gentle cleansers. Keep nails short and avoid picking scales. If a prescribed cream or other treatment causes marked burning, worsening redness, or a new rash, contact the prescriber for advice.

Photographing a changing lesion in consistent lighting can help you describe its progress at review. Also record joint pain, joint swelling, and morning stiffness, because the skin and musculoskeletal symptoms may need coordinated care. A physiotherapist can help if joint disease limits daily movement.

When to seek medical care

Arrange a dermatology or primary-care review for a new persistent lesion, rapidly spreading patches, painful cracks, or lesions that interfere with sleep or movement. Seek prompt care if the area becomes hot, increasingly painful, swollen, or produces pus, particularly with fever. A painful red eye or sudden severe joint swelling also needs urgent assessment.
