Psoriasis scales falling off: what to know

In psoriasis, inflammation speeds skin-cell turnover. Cells collect into a plaque and eventually loosen as flakes or larger scales. Falling scale may happen during a flare, after a scale-softening treatment, or as inflammation improves, but it does not by itself prove that the condition has cleared. The skin underneath may be red, delicate, or more sensitive to friction. Pulling attached scale can cause tiny injuries and bleeding. Similar shedding can accompany irritation, eczema, or infection, so an unfamiliar pattern deserves an examination.

Soften attached scale with the product recommended for that body area, then wash gently and pat dry. Apply a bland moisturiser to support the skin barrier. On the scalp, follow directions about contact time and keep treatment on the skin, not just the hair. Avoid scraping, picking, stiff brushes, and harsh exfoliants. Wash clothing and bedding regularly if loose flakes are bothersome, and note whether the affected area is enlarging, cracking, or becoming painful.

Arrange a review if scale keeps returning, covers new areas, or remains thick despite treatment. Seek prompt care for open cracks, repeated bleeding, yellow crust, pus, warmth, swelling, fever, or rapidly worsening pain. Widespread redness or peeling, or skin symptoms paired with swollen joints and stiffness, should be assessed without delay.
