What should you know about prolonged ovulation pain?

Typical ovulation pain is one-sided discomfort around the middle of a cycle and settles on its own. Pain that continues can instead reflect an ovarian cyst, endometriosis, infection, pregnancy-related problem, or another pelvic condition. Endometriosis often causes recurring pelvic pain linked with periods, sex, bowel movements, or urination. Ovarian cysts may cause pressure, one-sided pain, or sudden severe symptoms if complications occur. The timing of pain alone cannot confirm its cause.

Record the side, timing, severity, triggers, menstrual dates, bleeding, discharge, bowel or urinary symptoms, and any chance of pregnancy. Arrange a clinical assessment if pain lasts longer than your usual ovulation discomfort, keeps returning, or affects sleep and activity. A clinician may perform an examination, pregnancy test, infection tests, or ultrasound. Gentle heat and ordinary pain relief may help if suitable for you; check before taking medicines if pregnant or trying to conceive.

Seek emergency help for sudden severe or worsening pelvic pain, faintness, shoulder-tip pain, heavy bleeding, fever, repeated vomiting, or a positive pregnancy test with pain. These features can signal a problem that needs immediate examination, regardless of where you are in your cycle.
