Preseptal cellulitis stages

Early illness may look like localized redness, warmth, swelling, and tenderness of one eyelid. The area can become more painful or spread across nearby skin. A more advanced pattern is not simply a larger eyelid: pain with eye movement, reduced vision, double vision, or a protruding eye raises concern that infection has moved into the orbit. General illness may signal wider spread.

At the first new combination of eyelid swelling and redness, seek prompt clinical assessment rather than waiting for a stage to declare itself. Record when symptoms began and whether they are spreading. Use only treatment prescribed for this episode, complete it as directed, and attend follow-up so response can be judged.

Emergency assessment is needed for visual change, double vision, pain or difficulty moving the eye, a bulging eye, severe headache, vomiting, confusion, or rapidly expanding swelling. Fever with marked deterioration, faintness, or unusual weakness also needs immediate care.
