Pneumonia without a fever

Fever is one possible response to infection, not a requirement for pneumonia. Older adults, people with a weakened immune system and people taking medicines that affect temperature may have little or no fever. Early infection, a partially treated infection or a less typical pneumonia can also present differently. Shortness of breath, faster breathing, chest discomfort, unusual tiredness, confusion, chills, cough or phlegm may still occur. COVID-19 and influenza can overlap with these symptoms, so symptoms alone cannot identify the cause.

Arrange a clinical assessment if breathing feels harder than usual, symptoms are worsening, or you feel unusually weak despite having no fever. A clinician may listen to your chest and decide whether oxygen measurement, imaging or other tests are needed. Rest, drink fluids if you can, avoid smoke and follow prescribed treatment. Check temperature if you feel unwell, but also notice breathing rate, alertness, colour and ability to drink. Do not delay care because a thermometer reading is normal.

Seek emergency help for severe breathlessness, blue or grey lips, new confusion, fainting, chest pain, or difficulty staying awake. Get prompt advice for rapidly worsening cough, blood-stained phlegm, marked weakness or new chills. A serious infection, including sepsis, may not always produce a fever, so the whole pattern of symptoms matters.

Ask whether your symptoms need a chest examination, oxygen check or imaging, which changes should trigger urgent review, and how to take any prescribed medicines. Tell the clinician about immune conditions and medicines that can suppress fever.
