What should you know about pneumonia without a cough?

Cough is a common presentation of pneumonia, but it is not required. The infection may affect the lungs without producing much mucus, or a person may not be able to cough strongly. Older adults and people who are very unwell may show weakness, reduced appetite, fast breathing, or confusion instead. Some people feel pain when taking a deep breath or notice shortness of breath during ordinary activity. Fever and chills may occur, but their absence does not rule out a lung infection. A cough-free illness can resemble other conditions, including COVID-19 or influenza, so symptoms cannot identify the cause on their own. Examination and, when appropriate, oxygen measurement or imaging help guide the next step.

Seek medical advice if you have unexplained breathlessness, chest discomfort, fever, or a clear decline in energy, even without coughing. Describe when symptoms began, whether breathing is worse when walking or lying down, and any medical conditions or medicines. Rest, drink fluids if safe for you, and avoid smoke while arranging advice. Do not start leftover antibiotics, because treatment depends on the likely cause and your overall condition. Check in on children, older adults, or someone living alone if they seem unusually quiet or confused.

Get emergency help for severe breathing difficulty, blue or grey lips, fainting, new confusion, or inability to stay awake. Seek urgent assessment for severe chest pain, inability to drink, or rapid worsening even if there is no cough. A sudden change in alertness, breathing, skin temperature, or strength can signal sepsis and needs immediate care.
