Photophobia in one eye: the short answer
Photophobia in one eye can result from a surface irritation, inflammation, migraine-related symptoms, or another eye problem; new or painful symptoms need medical assessment. Light sensitivity means that ordinary light feels uncomfortable or causes a need to squint or close the eye. When it affects one eye, the difference between the two eyes can help a clinician identify the source.
What photophobia in one eye can mean
A red, gritty or watery eye may reflect irritation of the eye surface, including conjunctivitis. Light sensitivity with a severe headache, nausea or visual disturbances can occur with migraine. Eye pain, marked redness, blurred vision or difficulty keeping the eye open can point to inflammation or a problem that needs prompt examination. A foreign body, contact-lens irritation or a recent injury can also affect one eye. Rarely, light sensitivity accompanies a serious infection such as meningitis, especially when it occurs with fever, a strong headache, neck stiffness, confusion, vomiting or a new rash. Photophobia itself does not identify the cause, and a common symptom does not by itself establish a diagnosis.
What to do now
Move away from harsh light and use sunglasses outdoors if they make you more comfortable, but avoid staying in complete darkness for long periods. Do not rub the eye. If you wear contact lenses, remove them and do not put them back until the eye feels well and a clinician has advised you if symptoms continue. Rinse the eye with clean water if dust or a loose irritant may be present; do not try to remove an embedded object. Arrange an eye or medical assessment for symptoms that are new, persistent, recurrent or associated with redness, discharge, pain or a change in vision. When to seek care urgently
Seek urgent medical care if light sensitivity comes with sudden vision loss, a new severe eye pain, an injury, chemical exposure, a very red eye, a new irregular-looking pupil, or halos around lights with headache or vomiting. Contact-lens wearers with pain, redness or reduced vision should be assessed promptly because corneal infections can worsen quickly. Go to emergency care for photophobia with fever, severe headache, neck stiffness, confusion, fainting, weakness or a widespread rash. These warning signs matter even if the light sensitivity began recently or affects only one eye.
Questions for your doctor
Ask: Could the eye surface, cornea or internal parts of the eye be irritated or inflamed? Does my pattern of headache or visual symptoms fit migraine? Do I need an eye examination or a vision check? Should I stop contact lenses, eye makeup or any current eye drops? Which changes should prompt same-day review, and when should I return if the sensitivity does not settle?

Questions people ask
Is photophobia in one eye always an emergency?
No. Mild irritation can cause it, but urgent assessment is needed for severe pain, vision change, injury, marked redness or neurological symptoms such as fever and neck stiffness.
Can migraine cause light sensitivity in one eye?
Migraine can cause light sensitivity and visual symptoms, usually as part of a broader pattern. A new one-sided eye symptom, particularly with pain or reduced vision, still deserves medical assessment.
What should contact-lens wearers do?
Remove the lenses and avoid wearing them while symptoms continue. Pain, redness or reduced vision in a contact-lens wearer calls for prompt eye care.
