Persistent headache behind the eyes: what to know

Screen use, an outdated glasses prescription, dry eyes, poor sleep, or focusing for long periods can create aching around the eyes. Migraine may cause pain behind one eye with nausea or sensitivity to light. Sinus inflammation can cause facial pressure, especially with congestion, although facial pain has several possible explanations. A brain tumour or brain metastases can present with headache and neurological or visual symptoms, but pain behind the eyes alone does not establish either condition.

Notice whether the pain affects one or both eyes, whether eye movement changes it, and whether there is redness, discharge, halos, double vision, reduced vision, vomiting, weakness, or confusion. These details help separate an eye problem from a broader headache pattern.

Take regular screen breaks, use suitable lighting, drink fluids, and avoid rubbing irritated eyes. If you use glasses or contact lenses, follow safe wear and cleaning advice. Arrange a clinical or eye assessment when the pain persists, returns often, or affects reading, driving, sleep, or work. Bring your medicines and note visual symptoms rather than trying to diagnose the cause yourself.

Seek urgent help for sudden vision loss, severe eye pain with a red eye, new double vision, a sudden severe headache, weakness, confusion, seizure, repeated vomiting, or difficulty walking. Do not drive yourself if vision or alertness is impaired.

Ask whether an eye examination, vision check, or neurological examination is appropriate. Mention any new glasses, contact lens use, congestion, light sensitivity, or medicine changes, and ask which visual changes require same-day care.
