How peripheral involvement appears

Ankylosing spondylitis is an inflammatory arthritis best known for spinal and pelvic symptoms, but it can also affect peripheral joints such as the hips, knees, ankles or feet. Joint stiffness may follow rest, and morning joint stiffness can make the first part of the day difficult. Swelling or reduced range of motion may develop, sometimes on one side and sometimes in several areas. A clinician will consider the full pattern, including back symptoms, tendon pain, eye inflammation, skin changes and bowel symptoms.

Managing movement and symptoms

Book an evaluation when recurring stiffness, swelling or loss of movement interferes with work, walking or sleep. Bring a list of affected joints and note whether movement improves or worsens symptoms. Treatment may include prescribed anti-inflammatory medication and a rehabilitation plan. Exercise therapy can maintain joint mobility and strength when adjusted to the current flare. Physiotherapy may address posture, walking and joint protection; hydrotherapy can provide a lower-impact setting for movement if land exercise is difficult.

Warning signs

Seek prompt care for a hot, intensely painful swollen joint, fever, sudden inability to move a limb, or new severe eye pain with light sensitivity or blurred vision. These symptoms may need urgent assessment. Tell your clinician if weakness, numbness, bowel or bladder changes, or rapidly worsening back or leg symptoms appear.
