Perioral dermatitis from kissing: what to know

Perioral dermatitis is an inflammatory facial rash, not an infection passed between partners. Moisture from saliva can dry or irritate the skin as it evaporates, while rubbing and pressure may disturb the skin barrier. Lip balm, fragrance, cosmetics, beard products or toothpaste residue can also transfer during close contact.

A flare appearing after kissing does not prove that kissing caused the condition. Small bumps, redness, scaling or burning can have other triggers, and the rash may resemble pimples.

Keep the area clean with lukewarm water and avoid licking, scrubbing, picking and fragranced products. Gently pat away saliva rather than rubbing. If a partner uses a strongly scented lip or facial product, consider avoiding contact with that product while the rash settles.

A clinician can confirm the diagnosis and may prescribe topical antibiotic therapy or oral antibiotics. Do not use a steroid cream around the mouth without guidance, because topical corticosteroids may worsen perioral dermatitis.

Arrange an appointment if the rash persists, spreads, recurs after improvement or reaches the eyelids. Seek prompt care for eye pain, vision changes, facial swelling, fever, pus or rapidly worsening redness.
