Period-like cramps during ovulation

Ovulation can cause a cramp or ache when an egg is released from an ovary. It may be felt on one side and can vary from cycle to cycle. The timing can overlap with changes in cervical mucus or a small amount of spotting, but these features do not prove that ovulation is the cause.

Similar pain may occur with endometriosis, adenomyosis, fibroids, an ovarian endometrioma, pelvic inflammatory disease, or a bowel or urinary problem. If you use a combined pill or hormonal coil, hormonal changes and bleeding patterns may differ from an untreated cycle, so include contraception in your history.

Mark the pain on a calendar alongside bleeding, discharge, sex, bowel symptoms, and the suspected ovulation day. Heat, rest, and gentle movement may help a mild episode. Check with a pharmacist or clinician before taking pain relief if pregnancy is possible or you have a medical condition that affects its safety. If pain repeats, becomes stronger, or interferes with normal activities, arrange a review.

Get urgent care for sudden intense pelvic pain, fainting, heavy bleeding, fever with feeling very ill, repeated vomiting, or a swollen or rigid abdomen. Seek prompt medical advice for pain with unusual discharge, pain during sex, bleeding between periods, or a pattern that lasts longer than your usual mid-cycle discomfort.
