Period-like cramps in the middle of your cycle

Some people feel a one-sided ache or brief cramp around the middle of the cycle as an ovary releases an egg. It may be mild and settle without treatment, but timing is only a clue. Pain can also arise from endometriosis, adenomyosis, fibroids, an ovarian endometrioma, pelvic inflammatory disease, bowel problems, or the urinary tract.

Notice whether pain is always on the same side, lasts beyond the expected window, follows sex, comes with unusual discharge or fever, or affects your work and sleep. A symptom page about painful periods may help you describe a broader pattern, but it cannot establish the cause.

Keep a cycle diary with the first day of bleeding, pain location and intensity, duration, discharge, bowel or urinary symptoms, and contraception. Gentle heat, rest, and light movement may ease a mild cramp. Ask a pharmacist or clinician which pain relief is suitable, particularly if you are trying to conceive, might be pregnant, or have stomach, kidney, or bleeding problems. Do not use repeated self-treatment to delay review of recurring pain.

Seek urgent care for sudden severe pain, fainting, shoulder-tip pain, heavy bleeding, fever with feeling very unwell, repeated vomiting, or a rigid or swollen abdomen. Arrange a prompt appointment for recurring mid-cycle pain, pain during sex, unusual discharge, bleeding between periods, fertility concerns, or symptoms that steadily worsen.
