Period during early pregnancy: what it can mean

Some bleeding can happen when the cervix becomes more sensitive, including after sex or an examination. Bleeding can also occur with an early pregnancy loss. An ectopic pregnancy develops outside the main cavity of the uterus and may cause bleeding with one-sided pelvic pain, shoulder-tip pain, dizziness, or fainting. Pelvic inflammatory disease can affect reproductive organs and may be relevant when bleeding occurs with pelvic pain, unusual discharge, or fever. The amount and colour of blood do not identify the cause on their own.

Contact a pregnancy or gynaecology service promptly and explain when the bleeding began, how it is changing, whether you have pain, and the date of your last period. Use pads so you can observe the flow, and avoid putting anything into the vagina until a clinician advises you. Assessment may include pregnancy hormone tests and a transvaginal ultrasound to check the pregnancy location and development. If an ectopic pregnancy is found, treatment depends on its size, location, symptoms, and test results; salpingectomy is one surgical option when the affected fallopian tube needs removal.

Go to emergency care immediately for severe or worsening abdominal or pelvic pain, pain mainly on one side, fainting, marked dizziness, shoulder-tip pain, or bleeding that is rapidly soaking pads. These symptoms can indicate internal bleeding or another urgent problem. Seek same-day advice for any bleeding during a confirmed or possible pregnancy, even when pain is mild.

Ask what tests will confirm the pregnancy location, whether repeat hormone testing or ultrasound is needed, which pain relief is suitable, and which changes in bleeding should prompt an emergency visit.
