Perimenopause and spotting after your period

Short answer
Perimenopause can make bleeding less predictable, including light spotting after a period, but new, repeated, or heavy bleeding should be assessed.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common pattern
- Bleeding may become irregular during perimenopause.
- Track
- Date, amount, pain, discharge, and triggers.
What spotting may mean
During the menopause transition, ovarian hormone levels fluctuate rather than changing in a smooth line. The womb lining may therefore shed at an unexpected time, and a period may be lighter, heavier, earlier, or later than usual. Spotting can also come from the cervix, vaginal dryness, infection, contraception, pregnancy, or a growth such as a polyp. A pattern that is new for you matters more than the small amount of blood. Tracking the date, colour, amount, pain, and relation to sex can help a clinician understand it.
What you can do now
Record each episode and note whether it follows a missed or irregular period. Use a liner if helpful, and avoid inserting products or having sex if either causes pain or makes bleeding worse. If pregnancy is possible, take a pregnancy test and mention the result when seeking advice. Arrange a routine appointment if spotting recurs, follows sex, or comes with pelvic discomfort, unusual discharge, or urinary symptoms. Bring your medication and contraception list, including any hormone treatment.
When to seek care promptly
Seek urgent medical help if bleeding is heavy enough to make you feel faint, weak, breathless, or unusually unwell, or if it comes with severe pelvic pain. Get prompt advice for bleeding during a possible pregnancy, fever, or foul-smelling discharge. Do not assume spotting is hormonal simply because you are in perimenopause; a clinician may need to examine the cervix, check for infection, or investigate the womb lining.
Questions for your doctor
Ask what causes should be considered, whether you need a pregnancy test or examination, and how to monitor the bleeding. You can also ask whether your contraception or medicines may contribute and what changes should trigger urgent review.
